FULL-TIME PHYSICAL THERAPISTS ( PT)

Job Summary:

Evaluate and provide physical therapy services to home health patients with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments; fine motor, sensory-motor, and developmental delays; as well as traumatic injuries and swallowing disorders. Restore patient’s function, alleviate pain, and prevent disabilities by planning and administering medically prescribed physical therapy. Provides quality care by assessing and interpreting evaluations and test results. Determines physical therapy treatment plans in consultation with physicians or by prescription. Participate in care plan meetings, rehab meetings, and family training to communicate patient status during rehab. Other duties as assigned.

REQUIREMENTS:

A Bachelor’s degree in Physical Therapy or foreign degree equivalent and a valid State of Illinois Physical Therapist license.

Required Skills/Abilities:

1. Knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services.
2. Ability to remain calm in emergency or crisis situations.

Physical Requirements:

-Duties require extensive standing and walking.
-Requires lifting, positioning, pushing, and/or transferring patients.
-Duties require frequent reaching, stooping, bending, kneeling, and crouching.

Job Summary:

Evaluate and provide medically prescribed occupational therapy treatment to injured, ill, or disabled patients through the therapeutic use of everyday activities as a means to rehabilitate a muscle, or range of movement or otherwise improve a patient’s overall ability to perform lifestyle tasks. Help patients develop, recover, improve, as well as maintain the skills needed for daily living and working. Provide quality care by assessing and interpreting evaluations and test results. Determine occupational therapy treatment plans in consultation with physicians or by prescription. Participate in care plan meetings, rehab meetings, and family training to communicate patient status during rehab. Other duties as assigned

Job Summary:

Help patients develop, recover, improve, as well as maintain the skills needed for daily living and working. Provide quality care by assessing and interpreting evaluations and test results. Determine occupational therapy treatment plans in consultation with physicians or by prescription. Conduct training programs or participate in training occupational therapists in occupational therapy techniques and objectives. Assist in planning, directing, and coordinating occupational therapy programs as requested by the Clinical Director. Participate in care plan meetings, Rehab meetings, and family training to communicate patient status during rehab. Perform other related duties as assigned

REQUIREMENTS:

A Master’s degree in Occupational Therapy or Physical Therapy (or a foreign equivalent) and a valid Physical or Occupational Therapist license for the State of Illinois.
