      As a contract health agency, we provide nursing homes, supported living homes, and other medical facilities with the specific type of therapists they need to support their patients’ recovery and longevity. We place speech therapists, occupational therapists, and physical therapists in facilities where they can thrive — and help others thrive as well. JOB DUTIES: The Occupational Therapist evaluates and provides family-oriented services to the home health patient with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments; fine motor, sensory motor, and developmental delays; as well as traumatic injuries and swallowing disorders. This includes patient and caregiver education; discharge planning; and consultation to other professionals, clinics, or agencies. Essential Duties and Responsibilities: Evaluation and treatment of residents who demonstrate disorders or neurological deficit, weakness, and paralysis.  Evaluation and treatment will only be conducted after a written order is received from the physician. Provide initial and ongoing assessment to determine level of functioning.

JOB DUTIES: The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.

  • Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients functional needs and adjusts care plans to ensure optimal patient outcomes.

  • Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.

  • Conducts therapy practice within defined standards of care.


speech pathology (slp)

JOB DUTIES: Evaluate levels of speech, language, or swallowing difficulty. Identify clients' goals for treatment. Create and carry out an individualized treatment plan that addresses specific functional needs. Teach clients how to make sounds, improve their voices, and maintain fluency.

  • Provide aural rehabilitation for individuals who are deaf or hard of hearing.

  • Provide augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders, such as autism spectrum disorder or progressive neurological disorders.

Required Master’s Degree in Occupational Therapy/Physical therapy/Speech Pathology or foreign degree equivalent or a Bachelor’s Degree in Occupational Therapy/Physical therapy/Speech Pathology/ or foreign degree equivalent plus 5 years of progressive, post-baccalaureate Occupational Therapist/Physical therapy/Speech Pathology licensed experience.  Employee must also hold a valid State of Illinois license in Occupational Therapy/Physical therapy/Speech Pathology.
