Positions Available:

Full-Time Occupational Therapist in Arlington Heights, IL.

JOB DUTIES: The Occupational Therapist evaluates and provides family-oriented services to the home health patient with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments; fine motor, sensory motor, and developmental delays; as well as traumatic injuries and swallowing disorders. This includes patient and caregiver education; discharge planning; and consultation to other professionals, clinics, or agencies. Essential Duties and Responsibilities: Evaluation and treatment of residents who demonstrate disorders or neurological deficit, weakness, and paralysis.  Evaluation and treatment will only be conducted after a written order is received from the physician. Provide initial and ongoing assessment to determine level of functioning. Gather background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient’s physical and psychosocial functioning. Develop and revise the plan of care in consultation with the physician and other care-team members. Evaluate outcomes. Educate patients and/or families about proper safe use and recommend methods for installation of adaptive equipment. Completion of all necessary documentation, which includes but is not limited to the following items: Initial Evaluation Forms, Weekly Clinical Progress Notes, Monthly Recertification Forms, Plan Discharge Orders and Summary, All other required forms as designated by the Petitioner and Supervisor; Attend all required meetings and in-services, including Medicare meetings, patient staffing, family meetings, care plans. Develop programs appropriate to the needs of the patients, to assist them in achieving their highest feasible level of functioning. Assist in meeting the standards necessary to obtain state, federal, and other appropriate certification. Employ and follow all appropriate company infection control policies and procedures and will respect and follow Patient’s Rights Guidelines. Responsible for the appropriate scheduling of residents on caseload in addition to all required in-services and meetings. Scheduling and completing of in-services for facility staff education. Present her/himself in a professional manner at all times including professional conduct, professional communication, and appropriate dress while in the facility. Other duties as assigned by the Clinical managers or supervisors.

Required Master’s Degree in Occupational Therapy or foreign degree equivalent or a Bachelor’s Degree in Occupational Therapy or foreign degree equivalent plus 5 years of progressive, post-baccalaureate Occupational Therapist licensed experience.  Employee must also hold a valid State of Illinois license in Occupational Therapy.
